Our Specialist Support Coordination, Support Coordination, and Recovery Coach services are delivered by a team that has in-depth knowledge of providers in Melbourne and are experts at presenting you the choice of a diverse range of supports, ranging from weekend activities to allied health services and mainstream supports.

Psychosocial Recovery Coach

A Psychosocial Recovery Coach (PRC) utilizes their own lived experience of mental health and embodies it into the practice of a support coordinator. Our recovery coaches will work with you to define and develop your care plan which to help springboard your NDIS plan coming to life. By tapping into their knowledge of accessing healthcare, services and supports combined with peer support background we are ready to help you identify and meet their goals.

A recovery coach identifies themselves as a Peer and collaborates with the you chosen members of your care team to meet individual needs and situational requirements by determining aspects of the care plan, identifying, developing and implementing pathways to help you meet your goals, assessing progress and adapting or changing the plan as needed.

A PRC stands alongside you to help strengthen and apply self-determination in all aspects of your life and when you are not feeling 100%, we'll do our best to help you get there.

Support Coordination

A Support Coordinator works with you to identify what NDIS and public (mainstream) health systems have to offer you and use it to empower you and reach your goals. Once you have identified which options appeal to you, our team will work with you to engage with these providers.

Along the way, we would help you to understand your NDIS plan so that one day you are able to independently implement the supports you receive.

Specialist Support Coordination

Specialist Support Coordination is typically funded for complex participants who have exceptionally high needs or are in a crisis situation. Often there are matters that are very time-sensitive and where the participant may face additional and high-risk barriers to support. Additionally, coordinators must be provided by a professional with an Allied health background, which is why all our Specialist Support Coordinators have extensive experience in the mental health sector.

A Specialist Support Coordinator's role entails all the activities outlined in "Support Coordination" but addresses situations where it is appropriate to have a specialist deliver Support Coordination-style tasks, necessitated by specific high-level risks in the participant's situation.
